Position Overview

The Physical Therapist will provide skilled physical therapy services to patients in their homes, helping individuals improve mobility, strength, balance, coordination, and overall functional independence.

This is an excellent opportunity for a new graduate Physical Therapist to begin their career in home health while gaining valuable hands-on experience and working with a supportive care team.

Responsibilities

• Evaluate patients' physical abilities, mobility, strength, balance, and functional needs.
• Develop individualized treatment plans based on each patient's condition and goals.
• Provide skilled physical therapy interventions in the patient's home.
• Help patients improve mobility, strength, balance, coordination, and independence.
• Educate patients and caregivers on therapeutic exercises, mobility techniques, safety, and home exercise programs.
• Monitor patient progress and modify treatment plans as appropriate.
• Communicate effectively with patients, families, physicians, and other members of the interdisciplinary care team.
• Complete accurate and timely documentation of evaluations, treatments, and patient progress.
• Follow professional standards, agency policies, and applicable regulatory requirements.
• Promote patient safety and encourage independence within the home environment.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current New York State Physical Therapist license or eligibility for licensure.
• New graduates are encouraged to apply!
• Strong clinical, communication, and interpersonal skills.

• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Strong organizational and time-management skills.
• Reliable transportation and ability to travel to patients' homes.
