You will regularly reassess progress, adjust plans of care, and coordinate closely with an interdisciplinary team to ensure high-quality, patient-centered care. Communication with nurses, occupational and speech therapists, social workers, and physicians is a key part of the role, helping to coordinate services and achieve optimal outcomes. You can expect to manage a home health caseload with visits scheduled throughout the day, typically during daytime hours, with potential flexibility depending on organizational needs. Documentation will be completed using an electronic medical record system, and you will be responsible for maintaining accurate, timely records in accordance with regulatory and professional standards.
