Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings while developing individualized treatment plans
• Implement physical therapy interventions targeting mobility, strength, balance, and functional capabilities
• Collaborate with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Educate patients and their families on therapeutic objectives, discharge planning, and home exercises
• Maintain accurate documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in care team rounds and interdisciplinary discussions to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited program

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., neurology, geriatrics) are advantageous but not mandatory

Skills

• Prior experience strongly preferred
