Home Health Physical Therapist Assistant

CenterWell Home Health is seeking a compassionate Home Health Physical Therapist Assistant to provide one-on-one rehabilitative care to patients in their homes. Working under the direction and supervision of a licensed Physical Therapist, you will help patients regain strength, mobility, and independence following illness, injury, or surgery. In this role, you will travel to patients’ residences, implement individualized treatment plans, and deliver evidence-based therapeutic interventions such as gait training, therapeutic exercises, balance and coordination activities, and functional mobility training. You will observe and document patient progress, communicate clinical findings to the supervising Physical Therapist and interdisciplinary team, and support timely updates to the plan of care. You will educate patients and caregivers on home exercise programs, safe mobility, fall prevention, and use of assistive devices to promote safety and long-term success in the home setting. Drawing on CenterWell’s culture of compassion, integrity, and excellence, you will prioritize patient safety and quality outcomes, adhere to all regulatory and agency guidelines, and maintain accurate, timely clinical documentation. Collaboration is central to this position; you will work closely with nurses, therapists, and other healthcare professionals to coordinate visits, support discharge planning, and ensure a seamless care experience.
