Position Overview We are seeking an experienced, self‑driven full‑time Physical Therapist to join our dedicated Home Health team and provide exceptional care throughout the region. You will evaluate patients’ health, create and implement care plans, and track progress while working closely with the Clinical Director, Clinical Staff, and independently in the field. Responsibilities Provide, coordinate, and oversee skilled intermittent care to patients, including instruction and education of patients and/or caregivers to promote maximal functional status in the patient’s home. Regularly evaluate and re‑evaluate patient status. Administer treatments, monitor patient conditions, and report any signs or symptoms indicating a change in patient condition to Physician, Case Manager, and/or Director of Nursing as needed. Initiate, develop, implement, and revise the patient’s Plan of Care (POC). Address all problems identified in the POC. Provide educational information to the patient, family, or caregiver regarding knowledge deficits or needs related to the patient’s condition. Communicate with other disciplines involved in the case as warranted. Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device. Comply with all company policies and procedures to ensure high standards of care. Requirements Current PT License to practice in California – Required. Minimum 1 year of experience in Home Health or Hospice care – Required. Current California Driver’s License. Must be able to travel within a 15‑25 mile radius on a regular basis.
