Field Registered Nurse

Field Nurse (RN)The Field Nurse (RN) is responsible for conducting nursing assessments, developing care plans, supervising Home Health Aides (HHAs), and ensuring compliance with all New Jersey State regulations and agency policies. The Field Nurse serves as a liaison between clients, caregivers, physicians, and agency staff to ensure the delivery of quality home care services while promoting client health, safety, and independence.Essential Duties and ResponsibilitiesClinical Assessments & Care PlanningConduct initial client assessments and reassessments in the home environment.Develop, implement, and update individualized Plans of Care based on client needs and physician orders when applicable.Evaluate clients' physical, emotional, and environmental conditions.Identify changes in client status and communicate findings to appropriate parties.Provide recommendations regarding services, safety concerns, and care needs.Home Health Aide SupervisionPerform supervisory visits of Home Health Aides in accordance with New Jersey regulations and agency policy.Evaluate aide performance and provide coaching, training, and corrective guidance as needed.Ensure aides are delivering services consistent with the established Plan of Care.Complete and maintain documentation of supervisory visits and competency evaluations.Client & Family EducationEducate clients and family members regarding disease processes, medications, safety precautions, and available community resources.Provide guidance on maintaining independence and improving quality of life.Address client and family concerns professionally and promptly.Care CoordinationCollaborate with physicians, hospitals, case managers, caregivers, and community agencies.Communicate significant changes in client condition to the Director of Nursing and appropriate healthcare providers.Participate in interdisciplinary care discussions when necessary.Assist with transitions of care and service coordination.Documentation & ComplianceComplete all assessments, supervision notes, care plans, and clinical documentation accurately and within required timeframes.Maintain compliance with New Jersey Department of Health regulations and agency policies.Ensure client records are complete, accurate, and audit ready.Participate in quality assurance and performance improvement activities.QualificationsRequiredActive and unrestricted Registered Nurse (RN) license in the State of New Jersey.Graduate of an accredited school of nursing.Minimum of one (1) year nursing experience.Valid driver's license, reliable transportation, and current automobile insurance.Ability to travel throughout assigned service areas.Strong clinical assessment and documentation skills.Excellent communication and interpersonal abilities.Proficiency with electronic medical records and basic computer applications.PreferredHome care, home health, hospice, or community health experience.Experience supervising Home Health Aides or caregivers.Bilingual (Spanish, Creole, or other languages) preferred.Physical RequirementsAbility to travel between client residences.Ability to stand, walk, bend, lift, and carry up to 25 pounds.Ability to perform home visits in various residential settings.Ability to respond to emergencies and evaluate client conditions effectively.Work EnvironmentPrimarily field-based with travel throughout assigned counties.Occasional attendance at office meetings, in-services, and training sessions.Exposure to varying home environments and conditions.CompetenciesClinical ExcellenceAttention to DetailTime ManagementCustomer ServiceCritical ThinkingProfessional CommunicationRegulatory ComplianceProblem SolvingTeamwork and CollaborationHealth, Vision, Dental BenefitsCVS Virtual Care: Accessible Care, when and where people need it, includes Mental Health CounselingTeladoc- 24/7 Doctor Support (phone or video)Voluntary Life InsuranceCritical Illness, Group Hospital Indemnity, Accident InsuranceLegalShieldIDShieldPet InsuranceFlexible Spending Account/Health Savings AccountCommuter BenefitsEmployee Assistance Program/Health Concierge ServicesAbout Us:Our mission is to provide and restore client dignity and independence in their homes through individualized care plans in an effort to reduce caregiver role and stress on family members while avoiding nursing homes and assisted living facilities.Eminence Home Care, an affiliate of Ultimate Care, is a licensed home care provider located in Connecticut, Maryland, New Hampshire, New Jersey & Pennsylvania.
