This hybrid role includes 2–3 days in the field and 2–3 days in the office, allowing for flexibility while maintaining meaningful, in-person member engagement.What You'll Do:Serve as the primary point of contact for members with behavioral health and psychosocial needsConduct comprehensive assessments and develop person-centered care plansCoordinate care across behavioral health providers, medical teams, and community resourcesConduct in-person home and community visitsSupport transitions of care and post-discharge follow-upProvide education, coaching, and trauma-informed supportMaintain accurate documentation and meet program requirementsRequirements:Education: Master's degree in Social Work or Psychology requiredLicensure: Active Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, or Licensed Professional Clinical Counselor requiredExperience: 1+ years in care management, behavioral health, or related setting (Medi-Cal/ECM preferred)Bilingual English/Spanish preferredStrong communication and care coordination skillsKnowledge of behavioral health, SDOH, and community resourcesComfortable with field work and home visitsValid driver's license, insurance, and reliable vehicleBenefits:Medical, dental, and vision insurancePaid time off + holidaysQuarterly bonusesHybrid flexibilityGrowth and development opportunities
