Now Hiring: Registered Nurse (Part-Time Care Coordinator)
- Compensation Rate: $25 per hour - $25 per hour
We are currently hiring for a Registered Nurse to fulfill a Part-Time RN Care Coordinator position! This position offers the flexibility of working remotely from home, with the exception of scheduled in-home assessments to build your client base. Each client will receive a minimum of 4 in-person assessments each year.
As an RN Care Coordinator , you would provide the following services for each client:
- Act as a primary point of contact for the client / family for nursing related questions/ concerns.
- "On-duty" hours for this position will be during hours of 7a-7p, during which time the client / family can contact you as needed. We do ask that you are readily available to take calls/emails during "on-duty" hours, meaning that you are able to respond in no more than 30-minutes to each inquiry.
- Outside of your "on-duty" hours, communication will be limited to emergency situations only.
- Conduct the initial consultation and 90-day reassessments, including nursing assessment and care plan.
- Outside of the in-person assessments, the remainder of the job can be completed from home. Initial assessments are typically done within 24-48 hours of request, so you will need to have some schedule flexibility to accommodate short notice requests.
- Communicate with and supervise all CNA / GNA caregivers , providing direction, instruction, and mentorship as needed.
- Attend our monthly company meeting (virtually or in-person)
- You will be tasked to manage the care process, instruct caregivers, find solutions to issues and concerns that family members have related to your client(s) plan of care, and generally ensure that the care process runs smoothly. The main goal of this position is to ensure the success of the relationship between the caregiver(s) and the client. Solve problems where they exist, provide coaching and training, and support both parties emotionally and strategically.
Compensation
- You will be paid $100 per case on a bi-weekly basis . This covers all aspects of managing the case. You will start with up to 3-5 cases and increase as desired.
- You will be paid $50 per in person assessment for travel.
