RN Hospice Admissions - Weekends

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated admissions registered nurse (RN) to support patients and families throughout Palm Beach County. This role offers a consistent weekend‑only schedule—every Friday, Saturday, and Sunday from 9:00am to 9:30pm, making it an excellent opportunity for nurses seeking a three‑day workweek, work‑life balance during the week, or an alternative to traditional weekday schedules. What You'll Do Serve as the primary nurse responsible for evaluating patients referred for hospice services Educate patients and families on hospice care and clearly communicate all components of our continuum of care Collaborate with referring physicians and liaisons to coordinate individualized, hospice‑appropriate plans of care Act as a key liaison between Trustbridge Hospice and hospitals, physicians, long‑term care facilities, and other healthcare professionals Promote and advocate for the hospice philosophy through education and relationship‑building within the healthcare community Why Join Empath Health? Current Basic Life Support Certification. Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred. Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs Basic computer skills and EMR software experience a plus. Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
