Pay Rate:
$33.00 per hour
Schedule:
Monday–Friday | 9:00 AM – 5:00 PM
No nights or weekends

This role is open exclusively to New York State–Licensed Practical Nurses (LPNs) .

Job Overview:
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a team providing community-based mental health services in Ulster County, NY. This role supports adults with serious mental illness through a collaborative, mobile care model.
This is a great opportunity for LPNs looking for a weekday schedule, meaningful work, and a team-focused environment.

LPN Responsibilities:

• Provide outreach and nursing support to clients in the community
• Administer and monitor medications in coordination with a Psychiatrist
• Assist with mental and physical health assessments
• Coordinate referrals and follow up on care plans
• Maintain accurate and timely clinical documentation
• Participate in team meetings and interdisciplinary care planning

Qualifications & Requirements:

• Active New York State Licensed Practical Nurse (LPN) license required
• Strong clinical and patient assessment skills
• Ability to work independently in a community-based setting
• Valid driver’s license required
• Strong communication and teamwork skills
