A-Line Staffing is now hiring a Transition Specialist to support members throughout Northeast Ohio, including Cleveland, Akron, and Canton. This position offers the opportunity to work with a Fortune 500 healthcare organization and contribute to meaningful community-based healthcare initiatives, with potential for career growth.

This is a full-time, 6-month contract opportunity with the potential for extension based on demonstrated cost savings.

Transition Specialist Compensation

• Pay Rate: $19.00 per hour
• Schedule: Monday–Friday, 8:00 AM–5:00 PM EST
• Hours: 40 hours per week
• Location: Northeast Ohio, including Cleveland, Akron, and Canton
• Work Arrangement: Primarily remote documentation with approximately two in-person trips per week to skilled nursing facilities and Social Security Administration offices
• Mileage: Reimbursed for work-related travel
• Assignment Duration: 6 months, with potential for extension based on business needs and proven cost savings
• Benefits: Eligible A-Line Staffing employees may qualify for benefits after 90 days
• 401(k): Eligible for a 401(k) with company match after one year

Highlights of the Transition Specialist Role

• Work directly with members in skilled nursing facilities and coordinate with healthcare professionals and social workers.

• Help expand a statewide initiative focused on improving member outcomes and reducing healthcare costs.

Transition Specialist Responsibilities

• Review skilled nursing facility census information to identify members who may benefit from assistance with disability applications.
• Interview members and their family members to gather information necessary for SSDI applications.
• Obtain required signatures and collect relevant personal, medical, and functional information.
• Review electronic health records (EHRs) and gather supporting medical documentation.
• Communicate with members who may experience speech, language, or other communication difficulties.
• Coordinate with skilled nursing facility staff and social workers to obtain necessary information.
• Prepare, organize, and submit applications and supporting documentation to the Social Security Administration.
• Visit local SSA offices to deliver applications and paperwork as needed.
• Track application progress, respond to requests for additional information, and complete timely follow-ups.
• Maintain accurate, detailed documentation while managing assigned cases independently.

Transition Specialist Requirements

• Bachelor's degree or equivalent relevant experience demonstrating the necessary knowledge and skills.
• Approximately 2–4 years of related professional experience.
• Strong written and verbal communication skills.
• Ability to read and interpret medical records and understand activities of daily living (ADLs).
• Strong organizational skills and attention to detail.
• Ability to prepare detailed reports and maintain accurate documentation.
• Comfortable working independently and troubleshooting issues as they arise.
• Ability to travel locally to skilled nursing facilities and Social Security Administration offices.
• Ability to obtain SOAR certification if not already certified.

Preferred Qualifications

• Experience in social work, nursing, or skilled nursing and long-term care facilities.
• Experience with SSDI applications, disability benefits, or SOAR-related processes.
• Familiarity with electronic health records and clinical documentation.
• Experience working directly with members or patients from diverse backgrounds.
• Experience communicating with individuals who have speech or language difficulties.
• Existing SOAR certification.
