IND123 Responsibilities Provide clinical services to adult clients with mental health concerns; screen for behavioral health and substance use issues as appropriate. Develop and implement individualized treatment plans, goals, and discharge planning in collaboration with clients and care teams. Coordinate with multidisciplinary teams, manage referrals, and ensure continuity of care. Maintain accurate documentation and comply with program requirements and professional standards. Qualifications Bachelor’s or Master’s degree in Social Work or Counseling. Licensed clinician in the relevant field.
