Field-Based Behavioral Health Care Manager
Molina Healthcare of Illinois
Molina Healthcare in Iowa seeks a care management professional who supports care coordination across the continuum and collaborates with a multidisciplinary team to ensure member progress toward outcomes and cost-effective care. You will assess behavioral health needs, develop and monitor care plans with members and providers, promote integration of services including LTSS and community resources, and participate in interdisciplinary meetings.
