Job Summary

Crescent Manor Care Centers is seeking a highly skilled and compassionate Physical Therapist (PT) to join our veteran therapy team.

As a Physical Therapist, you will provide high-quality rehabilitation services designed to improve patients' mobility, independence, and quality of life. You will work collaboratively with an interdisciplinary team in a dynamic and supportive environment focused on clinical excellence and patient-centered care.

• Evaluate patients in a timely manner according to state, payer, and facility requirements.

• Provide comprehensive physical therapy services, including evaluation, treatment planning, and implementation.

• Develop individualized treatment plans and goals based on each patient's needs.

• Communicate and collaborate with other disciplines and facility staff as indicated by the treatment plan.

• Provide routine documentation in accordance with Medicare, facility, and other payer guidelines.

• Participate in family conferences when requested.

• Record and submit all required documentation and treatment charges accurately and in a timely manner.

• Participate in consultations, education, and interdisciplinary meetings as needed.

• Support appropriate discharge planning and patient transitions.

• Promote patient safety, independence, dignity, and quality of life throughout the rehabilitation process.

• Current VT state license as a Physical Therapist.

• Strong evaluation, assessment, and clinical reasoning skills.

• Ability to develop individualized treatment plans and rehabilitation goals.

• Ability to work effectively as part of a dynamic and cooperative interdisciplinary team.

• Flexibility to distribute workload according to departmental needs and individual responsibilities.

• Ability to participate in consultations, teaching, and meetings with residents, staff, families, and other healthcare professionals.

• Strong communication and interpersonal skills.

• Ability to educate and counsel patients, families, and staff regarding the rehabilitation process.
