Direct Home Care Worker - Center Line

SUMMARY The Direct Home Care Worker is responsible for providing supportive services to participants in the home or community setting, during transport and appointments, or at the day health center. The PCA’s responsibilities for each participant are specified in the Direct Home Care Worker Care Plan. The following statements describe the essential elements, functions, and requirements of the position and are not an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job. SPECIFIC DUTIES AND FUNCTIONS The Direct Home Care Worker’s plan of care is based on the initial and ongoing assessments that are performed by a home care RNCM. The Direct Home Care Worker assists in providing and maintaining optimal physical and emotional comfort to frail elders in the participant’s home, community setting, day health center, dementia unit, and during transport and appointments. The Direct Home Care Worker assists participants and their families toward achieving maximum self-reliance and independence within their environment, day health center, and dementia unit. The Direct Home Care Worker performs the care or services outlined in the individual participant’s Care Plan. Tasks include: Personal care assistance Assistance with meal preparation and feeding Assistance with toileting and managing incontinence Simple treatments Shopping Medication reminders Laundry services Extended hours care Homemaking services and chores Transportation and off‑site appointment support Documentation responsibilities include accurate and timely record‑keeping for every visit or encounter, transport, and appointment. The Direct Home Care Worker keeps the RNCM and/or clinic nurse informed of the participant’s progress, immediately reports customer service complaints, deviations from the schedule, and appointments to the supervisor or manager. The Direct Home Care Worker responds to emergency situations per established policies and procedures and works only within the established scope of practice. KNOWLEDGE, SKILLS AND ABILITIES Minimum high school diploma or GED required. At least two years of relevant work experience or completion of a formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) in lieu of the experience requirement. The Direct Home Care Worker participates in annual required in‑service training and screening, including but not limited to: Infection control TB testing Safety training BLS training The role requires the ability to establish and maintain interpersonal and interdepartmental relationships, respect the physical, spiritual, and well‑being of participants and co‑workers, organize and maintain flexible scheduling, work sensitively with diverse ethnic and cultural backgrounds, relate to participants with dementia and developmental disabilities, and perform duties in an environment with frequent interruptions. The Direct Home Care Worker must meet standardized competencies approved by CMS before working independently and be medically cleared for communicable diseases with all immunizations up‑to‑date before engaging in direct participant contact. WORKING CONDITIONS Driving is required within the PACE Southeast Michigan catchment area. Reliable transportation with a current valid driver’s license is mandatory. Working conditions vary and include exposure to communicable diseases, cleaning solutions, and medications.
