Mental Health Case Manager

Duties

Perform difficult and complex assessments.

Evaluate and analyze health care plan and resources.

Communicate with treating facilities, both in direct and purchased care to coordinate patient care and transitions.

Monitor, track and intervene as patients transition between levels of care.

Develop and recommend strategies to improve healthcare access, reduce administrative barriers for beneficiaries, and enhance the efficiency of the MTF.
