Minimum Requirements

Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
Active Illinois Physical Therapist license required to start the assignment.
BLS certification from AHA or ARC may be required.
Home Health experience preferred.
Experience with OASIS documentation preferred.
Experience with HCHB (Homecare HomeBase) electronic records preferred.
Minimum education requirement: Bachelor’s Degree.

Assignment Details

Facility Type: Home Health.
Shift Name: Days.
Schedule: 4x8 (four 8‑hour shifts per week).
Weekly hour guarantee: 32 hours.
On Call/Weekend Requirement: No.
Coverage Areas: Clinton, Marion, Washington, Jefferson, and Hamilton counties; candidate should be prepared for travel between patient homes within these counties.
Mileage reimbursement provided between visits from first patient to last patient.
Required certification: Illinois Physical Therapy license.
Must provide in-home physical therapy evaluations and related skilled services.
