Job Summary

We are seeking an experienced LMSW Social Worker to support an adolescent mental health program. The Social Worker will provide direct clinical services to youth and work closely with an interdisciplinary treatment team to support each individual's mental health and treatment goals.

Responsibilities

• Provide individual, group, and family therapy to adolescents.
• Provide crisis intervention and support as needed.
• Participate in weekly treatment team and interdisciplinary meetings.
• Develop and support treatment plans based on client needs.
• Connect youth and families with appropriate community-based resources and services.
• Maintain accurate, timely, and complete clinical/medical record documentation.
• Assist with discharge planning and coordinate appropriate follow-up services.
• Communicate with internal staff, families, schools, and community service providers as appropriate.
• Monitor client progress and communicate relevant updates to the treatment team.
• Maintain confidentiality and follow applicable policies, procedures, and documentation requirements.

Required Qualifications

• Active New York State LMSW license required.
• Master's degree in Social Work.
• Experience providing mental health services, preferably with adolescents or youth.
• Experience with individual, group, and/or family therapy.
• Strong communication, documentation, and organizational skills.
• Ability to work effectively as part of an interdisciplinary team.
• Experience with crisis intervention and care coordination is preferred.
