Contract | Monday – Friday, 40 hours per week | Days | 13-Week Assignment

Key Responsibilities

• Provide physical therapy services to patients within an assigned home health territory
• Evaluate patients and develop individualized treatment plans to address functional deficits
• Document patient progress and clinical notes using HCHB (HomeCare HomeBase) EMR
• Coordinate care with interdisciplinary team members to support patient goals and outcomes
• Manage a caseload across the designated service territory during standard weekday hours

Required Qualifications

• Active Physical Therapist (PT) license in the state of Wisconsin
• Minimum of 12 months of home health experience within the past 3 years
• HCHB (HomeCare HomeBase) EMR experience required
• Ability to independently manage a patient caseload across a defined territory

Preferred Experience / Skills

• Experience working in a home health or community-based therapy setting
• Strong time management and self-directed work skills suited for field-based practice
