Active Living Orthopedic Facility is seeking a dedicated and skilled Orthopedic Physical Therapist to join our dynamic team in Thousand Oaks, California. In this full-time role, you will play a crucial part in guiding patients through their rehabilitation journeys, helping them regain their strength, mobility, and quality of life. As a vital member of our healthcare team, you will focus on delivering personalized care in an orthopedic outpatient setting, utilizing evidence-based practices to achieve optimal patient outcomes.

Work Environment

Our facility is an onsite rehabilitation center located in the heart of Thousand Oaks, California, surrounded by a supportive community that values health and wellness. The work environment is designed to encourage teamwork and continuous improvement among staff, with state-of-the-art equipment and designated treatment areas for patients. You will be working alongside two other physical therapists and one Physical Therapist Assistant (PTA), ensuring a collaborative approach to patient care.

Key Responsibilities

  • Conduct thorough evaluations and assessments of patients to develop tailored rehabilitation plans that address individual health goals and needs.
  • Manage a caseload of patients, typically handling one patient per hour, allowing for focused and individualized attention.
  • Implement a variety of treatment modalities, including manual therapy techniques and therapeutic exercises designed to improve patients' range of motion and functional abilities.
  • Educate patients and their families on injury prevention, rehabilitation strategies, and exercises to promote a healthy lifestyle.
  • Monitor patient progress and adjust treatment plans as necessary to meet evolving objectives.
  • Document all patient interactions, treatment modalities, and progress consistently and accurately using EMR software, specifically WebPT.
  • Collaborate with a multi-disciplinary team, consulting with other healthcare professionals to ensure comprehensive care tailored to the needs of each patient.
  • Motivate and encourage patients throughout their therapy journey, utilizing positive reinforcement and support to optimize treatment outcomes.
  • Assess treatment outcomes regularly and maintain open lines of communication with patients regarding their progress and adaptations to their care plans.

Patient Population / Client Focus

At Active Living Orthopedic Facility, you will primarily work with patients recovering from orthopedic injuries, surgeries, and chronic pain conditions. Our diverse patient population ranges from active adults to older adults needing specialized rehabilitation to regain independence and mobility. We prioritize a patient-centered approach, valuing each individual's unique journey and encouraging their active participation in the rehabilitation process.

Qualifications

  • A Master’s or Doctorate degree in Physical Therapy (DPT) from an accredited institution.
  • A valid state license to practice as a Physical Therapist in California.
  • Maintain an untarnished professional record without any history of board or disciplinary action.
  • Exhibit proficiency in manual therapy techniques and evidence-based therapeutic interventions.
  • Demonstrate strong organizational skills and attention to detail in handling patient care documentation.
  • Effective communication skills to enhance patient interactions and multidisciplinary collaboration.

Preferred Experience

  • Prior experience in an orthopedic outpatient setting, with a strong focus on rehabilitation practices.
  • Proficient use of Electronic Medical Records (EMR) systems, particularly WebPT.
  • Experience in implementing customized treatment protocols and fitness programs tailored to various patient populations.

Compensation and Benefits
