- Communicate patient needs and progress to the treatment team and family members
- Consult with medical providers regarding patient evaluations and progress
- Recommend and assist in securing adaptive equipment for patients
- Maintain and submit documentation as required
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development programs and obtain continuing education
- License as a Physical Therapist
- Home Health experience required
- HCHB experience preferred but not required
