Primary Care Registered Nurse (PRN & Outpatient)

Primary Responsibilities Key Responsibilities: Skills: Patient check-in Provide disease management Scrub appointments Increase patient access through MyHealthyVet/secure message responses Collaborate with community care Patient assessments Medication administration IV therapy Interdisciplinary team coordination Telemetry monitoring Admissions, discharges, internal and external transfers Delegating to assistive personnel Electronic medical records documentation Patient transport Emergency response procedures And all duties as assigned. Minimum Qualifications Requirements: Licensure: Current, unrestricted RN license from any state in the United States. Certification: Current Basic Life Support (BLS) certificate accredited by the American Heart Association (AHA). Experience: Minimum of four (4) years of current nursing experience to include at least two (2) years of inpatient experience in care coordination and at least two (2) years of nursing experience in an outpatient setting. Education: ASN accepted; BSN preferred. o Education must have been completed at a school accredited by the ACEN or CCNE.
