We are seeking a compassionate and detail-oriented Community Care Coordinator to oversee daily resident care, assist with Activities of Daily Living (ADLs), and manage caregiver scheduling. This role is perfect for a Certified Nursing Assistant (CNA) with strong leadership and organizational skills.

Assist residents with ADLs including bathing, dressing, grooming, toileting, and mobility. Plan and post monthly schedules for CNA, Caregiver, Med-Tech, and Nursing staff. Adjust staffing for call-offs and emergencies to ensure coverage. Support recruitment efforts by scheduling interviews and assisting with onboarding. Greet visitors, answer calls, and coordinate communication between residents, families, and staff. Monitor resident health, respond to emergencies, and document changes. Maintain accurate health records, medication logs, and supply inventories. Participate in resident care conferences, activities, and welcome/orientation programs. Ensure cleanliness and organization of resident rooms, dining areas, and common spaces. Qualifications:

High school diploma or equivalent. CNA Certification (required). CPR/First Aid Certification. Strong communication, computer, and organizational skills. Ability to work in a fast-paced, team-oriented environment. 12 hours of annual continuing education (provided in-house). Competencies:
