Contract | Full-Time | Days | Monday–Friday, 8-hour shifts | No weekends | No call | Onsite / Field-Based

Key Responsibilities

• Plan and conduct segments of physical therapy treatment programs designed to restore and improve patients' physical, social, and mental functions
• Meet departmental objectives while delivering high-quality, patient-centered care in a home health setting
• Serve as co-leader in an interdisciplinary approach to patient care and group treatment
• Complete required clinical documentation using HCHB and OASIS platforms accurately and in a timely manner

Required Qualifications

• Doctor of Physical Therapy (DPT) degree required if licensure was obtained on or after January 2016, or foreign equivalent
• Active Physical Therapy licensure in all applicable states of practice
• Current CPR or BLS certification from an approved vendor (AHA-BLS or AHA-CPR accepted)
• Valid Driver's License; must be at least 21 years of age
• Minimum of 1 year of home health experience
• Proficiency with HCHB and OASIS documentation systems

Preferred Experience / Skills

• Experience working within an interdisciplinary home health care team
• Strong organizational and time management skills suited for an independent, field-based work environment
