Case Manager (PRN)

Clarksville Rehabilitation Hospital is a brand new startup with 40 private patient suites and set to take its first patient October 2026. Position Summary: The Case Manager, in collaboration with the physician, providesindividual program management for each patient toensure the patient’s progression through the continuum of care in a manner that achieves the desired clinical andfinancial outcomes. Monitors and manages clinical and financial coordination of treatmentplanofassigned patientsto ensuretimely, cost-effective, individualized service delivery. Works with rehabilitation patients with variousdisabilities including, but not limitedto:spinal cord injury, brain injury, cerebrovascular accident, amputation,neurologic disorders, orthopedicconditions, and arthritis. Coordinates length of stay management within Medicare(CMS) guidelines and 60% compliance threshold. Provides community resources as appropriate including vocational rehabilitation resources. Pay:Rate of pay is based on years of experience and qualifications. Minimum Qualifications: Current state licensure as a Registered Nurse or Licensed Social Worker. Desired Qualifications: 2 years experience in a hospital setting preferred. Certification in case management preferred. Knowledge,Skillsand Ability Requirements: Excellent communication, negotiation, and conflict resolution skillsrequired. Knowledge of reimbursement systemspreferred.
