Confidential Opportunity – Social Services Director (Long-Term Care) A skilled nursing and rehabilitation facility is seeking an experienced Social Services Director to lead the Social Services department and support the psychosocial well‑being of residents and their families. Position Summary The Social Services Director oversees all aspects of resident social services, including admissions support, care planning, discharge planning, resident advocacy, and compliance with federal and state regulations. This leadership role partners closely with the interdisciplinary team to enhance resident outcomes and quality of life.
