Position Overview: Centria Healthcare is seeking compassionate and skilled Home Care Nurses to join our team in Detroit, Michigan and surrounding cities. As a Home Care Nurse, you will play a pivotal role in providing specialized nursing care to children and adults with complex medical needs within the comfort of their own homes. • 7:00am-3:00pm (8 hour shift)
• 8:00am-4:00pm (8 hour shift)
• 9:00am-5:00pm (8 hour shift)
• 10:00am-6:00pm (8 hour shift)
• 10:00pm-6:00pm (8 hour shift)
• 3:00pm-11:00pm (8 hour shift)
• 8:00am-6:00pm (10 hour shift)
• 10:00pm-8:00am (10 hour shift)
• 7:00am-7:00pm (12 hour shift)
• 11:00pm-7:00am (8 hour shift)
• 9:00am-9:00pm or 10:00am-10:00pm (12 hour shifts)

Nurse Centric Scheduling

We understand that our team of nurses have increasingly busy schedules and we want to do our best to prioritize YOU. With our new culture of nurse centric scheduling at Centria Healthcare, there are no mandated shifts and we are incredibly flexible with your traveling needs. Our nurses also work with our families to create personalized schedules in a way that works for all involved.

Responsibilities

• Perform duties outlined in patient's plan of care
• TBI + SCI rehabilitation
• Pediatrics
• Geriatrics
• Ventilator and Tracheotomy care (Training Provided)
• Gastrostomy tube care (Training Provided)
• Dispensing medications
• Observe client’s physical, mental and emotional conditions.
• Report any changes
• Document observed changes
• Communicates effectively with Clinical team to increase optimal outcomes
• Maintains professional nature within the home setting

Required Qualifications, Skills, and Knowledge

• Excellent oral and written communication skills
• Ability to follow verbal and written instructions
• Ability to utilize computers/tablets and computer software
• Current state license as an RN or LPN
• Valid driver’s license and reliable transportation
• Current negative TB skin test or negative chest X-ray
• Current flu vaccine
• CPR certified
