RN MDS Coordinator
Floresville Nursing and Rehabilitation – Floresville, TX

Floresville Nursing and Rehabilitation is hiring an RN MDS Coordinator ! You'll protect our accuracy, compliance, reimbursement, and work closely with a team that values what you bring. You would work a steady schedule in a close-knit community just southeast of San Antonio.

What You'll Do

• Coordinate, complete and submit accurate, on-time MDS assessments (admission, quarterly, annual, significant change, PPS and discharge) following the CMS RAI Manual

• Manage the assessment schedule so no deadline or window gets missed

• Complete Care Area Assessments (CAAs) and help build individualized, person-centered care plans

• Lead or take part in IDT and care plan meetings with nursing, therapy, dietary, social services and activities

• Check that clinical documentation supports what is coded, and fix gaps with the right team members

• Capture PDPM correctly so reimbursement reflects each resident's acuity and care

• Transmit assessments, review validation reports and fix errors quickly

• Keep up with CMS and Texas HHSC regulation changes, and help the clinical team with training and survey readiness

• Watch quality measures and work with the DON and Administrator on improvement plans

What You'll Need

• Active RN license in Texas or a compact multistate RN license (required)

• At least 2 years of experience as an MDS Coordinator in a skilled nursing facility (required)

• Strong working knowledge of the RAI Manual, the MDS 3.0 process, PDPM and care planning

• Experience with EHR/MDS software such as PointClickCare, MatrixCare or similar

• RAC-CT certification preferred

• Detail-oriented, organized and able to handle multiple deadlines

• Clear communicator who works well with an interdisciplinary team
