Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• At least 12 months of Home Health experience within the past 3 years (client requirement).
• HCHB (Homecare Homebase) EMR experience required.
• Ability to manage a productivity expectation of a minimum of 30 points per week (approximately 6 points per day).

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Monday-Friday, 8:00 AM-5:00 PM, 8-hour shift.
• Schedule: 8 AM-5 PM.
• Territory: Trumbull, Mahoning, and Columbiana counties (territory travel expected within assignment).
• Length of Contract: 13 weeks.
• On-call: No.
