Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join a travel Physical Therapist role in Albany, KY providing home health services. Perform physical therapy evaluation and treatment based on physician orders/referrals. Complete comprehensive evaluations using appropriate assessment tools. Establish measurable goals and develop treatment plans to move patients toward maximum independence and function. Re-assess treatment effectiveness and modify plans as needed. Communicate patient needs and progress to the care team, physicians, patients, and families. Recommend and assist in securing adaptive equipment and incorporate its use into rehab programs. Maintain and submit evaluation, treatment, and billing documentation per company, facility, funding source, and regulatory requirements. Adhere to applicable professional practice standards, HIPAA, company policies, and state and federal laws. Supervise Physical Therapist Assistants and Aides in direct patient care per state practice act. Participate in staff development and obtain continuing education as required.

Minimum Requirements

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky license or eligibility through the compact to begin the assignment.
• Home health experience required.
• Experience with HCHB and OASIS documentation required.
• Wound care experience required.
• BLS certification from AHA or ARC may be required.

Preferred Qualifications

• Prior travel home health experience preferred.
• Experience supervising PTAs and aides preferred.

Assignment Details

• M-F, 8:00 AM - 5:00 PM.
• Counties covered include Clinton, Cumberland, Monroe.
• License requirement: KY compact state.
• Mileage reimbursement: $0.43 per mile.
• No on-call schedule.
• May require travel within the county coverage area; floating to other nearby service areas possible.
