A Day in the Life of an ADvantage Home Care RN Case Manager

As an ADvantage Home Care RN Case Manager, you play a key role in ensuring members receive the services they need to remain safely in their homes. Your responsibilities include:

• Arranging and coordinating service plans
• Providing nursing assessments and education
• Collaborating with case managers and other care team members
• Ensuring documentation meets agency and ADvantage program standards
• Supporting members and families with professionalism, compassion, and respect
• Maintaining regular attendance and punctuality
• Home visits with members

Qualifications

• Active RN license in the state of Oklahoma
• No previous ADvantage experience required.
• CPR certification within 60 days of hire
• Valid driver's license and good driving record
• Reliable transportation with proof of insurance
• Ability to travel long distances and adapt to frequent schedule changes

Work Schedule

This full-time position typically operates Monday-Friday, 8:00 AM to 5:00 PM .
