Job Details Location: Topeka, KS 66606 Position Type: Full Time Shift: Day Overview Midland Care Connection is a not‑for‑profit, community‑based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All Inclusive Care for the Elderly) program. Hospital Home Care Our hospice program provides compassionate, comprehensive support for individuals facing terminal illnesses, helping them live with comfort, dignity, and peace in the place they call home. The program focuses on improving quality of life, addressing physical, emotional, spiritual, and social needs, and offering bereavement support to families after a loved one’s passing. RN Case Manager Responsibilities In the hospice program, an RN Case Manager coordinatively delivers high‑quality, patient‑centered end‑of‑life care. Responsibilities include assessing patients’ medical needs, developing individualized care plans, ensuring appropriate symptom management and medication administration, serving as the primary clinical liaison between patients, families, and the interdisciplinary hospice team, providing education and emotional support to families, navigating complex medical decisions, ensuring compliance with regulatory standards, and advocating for patients’ wishes to enhance quality of life. Sign-On Bonus $3,000 paid in two increments at 6 months and at one year of employment.
