Description Gratiot Integrated Health Network (GIHN) has an immediate opening for a full-time, Assessment Specialist.The Assessment Specialist is responsible for providing initial and/or additionally requested comprehensive assessments, determining appropriate treatment planning, and/or making referrals to other services if deemed necessary. The Assessment Specialist is involved in organized collaborative efforts that may require services from multiple systems. SALARY RANGE: $60,736-$86,049.60

EMPLOYMENT CLASSIFICATION: Regular, Full-Time, Exempt/Salary Status Requirements Essential Functions. Performs initial screeningand psychosocial assessmentof children, adolescents and adults requesting mental healthand/or substance abuseservices. Provides crisis intervention and access screen services to consumers by phone, walk-in or scheduled appointment and serves as back-up to the CrisisInterventionSpecialist. Conducts continued stay reviews atappropriate intervalson inpatient hospitalizations, partial hospitalizations, and crisis residentialunits todetermineif clinical information supports medical necessity for continued treatment. Coordinates post hospital discharge planningwith hospitals and GIHNstaff. Performs diagnostic evaluation in collaboration with the Medical Director, Clinical Director, ProgramSupervisor,and/or Director of Service Utilization as needed. Determineseligibility and level of care based on the practice guidelines of thePIHP. Performs internal program referrals based on eligibility requirements and provides theinitialauthorization for service(s). Provides ineligible applicants with referral recommendations and Recipient Rights informationregardingthe Agency’s Reconsideration of Eligibility process. Provides collaboration with referral sources such as physicians, schools, professionals,other agencies,hospitals, courts, etc. Participates in peer reviews/utilization reviewmeetingsas necessary. Effectively collects, reports, and uses data and information to support authorization decisions and to improvesupportsand services across the organization. May provide therapy ininitialsessions, aftercare follow-up, and/or crisis intervention services and act as a co-therapist with other clinicians, as needed. May serve as a member of the emergency services crisis team, providing24 hourcrisis intervention for the residents of Gratiot County. Establishes a positive and productive relationship with community agencies and residents in keeping with current developments in the mental health field. Protects the security,confidentialityand integrity of all protected health information in a manner that is consistent with HIPAA, MDHHS and GIHN policy and procedure. Assuresconsumers’ rights areprotectedand recipient rights procedures are followed throughout GIHN and its provider network. Attends and activelyparticipatesin GIHN required training as well as external education opportunities for professional growth and development. Abidesby all applicable GIHN policies and procedures. Maintains a safe and clean work environment. Performs work and otherduties asassigned by the supervisor. Competencies. Mustexhibitstrong spelling,grammarand both oral and written communication skills. Supervisory Responsibility. This position has no supervisory responsibilities. Work Environment. This joboperatesin a professional office environment. This role routinely uses standard office equipment such as laptop computers and smartphones. Physical Demands. The employee must occasionally lift or move up to 20 pounds. Position Type/Expected Hours of Work. This is a full-time, salaried, exempt position. Some flexibility in hours is allowed but the employee is expected to be available during the core works hours of Monday through Friday, 8 a.m. to 5 p.m. the majority of the time, and must work40 hourseach week to maintain full-time status, unless an adjusted schedule is pre-approved (a minimum of 30 hours per week is required to maintainfull timebenefit status). Some evening hours and weekends may berequired. Travel. Limitedto notravel is expected for this positionfrom the Alma Headquarters. Required Education and Experience. Master’sdegree in Social Work, Psychology, or related field,is required. Michigan licensureappropriatetoprofessional degree,i.e.LLP, LMSW, is required. Must have experience in thediagnosis andprovision of services to individuals with mental health illness, developmental disabilities, substance use disorders and co-occurring disorders in a mental health or other human services agency. Preferred Education and Experience. Three years of work experience in the mental health field. EEO Statement.
