Primary Care Nurse I (RN)Columbia University Irving Medical Center (CUIMC) is seeking an Registered Nurse to join our ColumbiaDoctors Primary Care team. The Primary Care Nurse will report directly to the Director of Nursing, Columbia Primary Care. This ambulatory nursing position will serve as a key member of the patient care team and will support a pediatric and adult population across assigned Primary Care practices.The Primary Care Nurse I is an entry level role and performs telephonic, electronic, and in-person nursing assessments consistent with the NYS nursing scope of practice. The role serves as a clinical lead in the practice with responsibility for the maintenance of infection prevention and control procedures in addition to precepting new clinical support staff.ResponsibilitiesAdministrative/OperationsExecutes nursing interventions by collaborating with the care team and following the standards of care established by Columbia Primary CareAssesses and systematically collects data related to patient health status and works with the care team to develop the plan of careUtilizes data to continuously meet care coordination needs across settings in conjunction with the patient, family, and/or other care providersAssumes responsibility and accountability for effectively managing nursing care of individual patientsDocuments nursing assessment and interventions to reflect a comprehensive and integrated approach to patient careIdentifies learning needs of the patient and families, educates by adapting to patient and family learning style utilizing the teach-back methodProvides care that is safe, ethical, patient- and family-centered, culturally congruent, and evidence-basedTelephone, My Chart, and in-person triage (for symptom management) under supervision as appropriateReview and act on test results (lab, x-ray)Review and manage communications from internal and external customers inclusive of inbasket messages, updating immunizations, updating health maintenance items.Participate in providing patient education through multiple avenues inclusive of in-person, by telephone, or videoExecute delegation protocolsAssist with medication management (e.g., medication renewals) inclusive of medication prior authorizations as needed. Administer immunizations as an adjunct to LPN staffComplete accurate and timely documentation in the electronic medical recordAssist with management of urgent and emergent patient care situationsAssist with the coordination of care across the continuum (e.g., inpatient, outpatient, home, skilled nursing facility, schools)Assist with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness. Perform patient outreach related to gaps in care.Monitor supply inventories for practice and order supplies as needed. Assists Supervisor and/or Manager with re-orders and inventory tracking.May float to other clinics as needed for business continuity. Occasionally provide assistance in the clinical duties of MA and LPN staff in the interest of patient careServe on relevant committees, task forces, and workgroups within Columbia Primary Care and the OrganizationOther duties as assigned appropriate to scope, education, and/or trainingPeoplePrecept clinical staff (primarily Certified Medical Assistants) and perform competency testing for new and established staff as appropriate.ComplianceMaintain patient privacy and confidentiality according to HIPAA requirements at all times.Keep current on all organizational and practice policies (e.g., infection control, HIPAA), goals, initiatives, and required trainingsEnsure that University and Department policies and procedures are followed.Successfully complete all required university, hospital, and department trainings.Other duties as assigned within professional scope of practice/training/education.This list of duties and responsibilities is not intended to be all-inclusive and may be expanded to include other duties or responsibilities as necessary.Minimum QualificationsBachelor of Science degree in Nursing or equivalent of education, training, and experience.A minimum of 2 years of related experience.An equivalent combination of education, training, and experience may be considered.Current and unrestricted New York State RN license.Basic Life Support certifiedAbility to work indoors in an environmentally controlled environment with a degree of dust, noise, odors, etc., without undue discomfortAbility to perform sedentary work that primarily involves sitting or standing for long periods of timeTravel between clinical sites may be required depending on operational needDuties performed routinely require exposure to blood, bodily fluid, and tissue. The incumbent is expected to perform and/or/assist in emergency care and/or first aid as neededAbility to work collaboratively with a culturally diverse staff and patient/family population demonstrating tact, respect, and sensitivity.Must have excellent communication skills and the ability to communicate effectively with a diverse audience of peers, management, and other stakeholders.
