Opportunity Advanced Practitioners - Primary Care Job Details Join a collaborative and well‑established Family Medicine team as a Nurse Practitioner (FNP) or Physician Assistant (PA) in a 100% outpatient Primary Care setting Supportive practice includes 4 physicians and 1 APP, an embedded behavioral health provider, and a clinical pharmacist working together to deliver comprehensive, patient‑centered care Full‑time position with weekdays only schedule - no call required Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients Develop plans for further evaluation of health needs, including referrals to other health team members or specialists Provide appropriate treatment for problems within the scope of practice and identify issues requiring physician referral Family Medicine panel serving patients from birth and older Work within an outstanding team culture known for efficient workflows and strong collaboration between providers, nursing, medical assistants, and front office staff Unrestricted State of Connecticut Medical License required (or eligible) APRN (Family Nurse Practitioner) or PA training required Minimum 1+ years of experience in a primary care or relevant outpatient setting required Access to dedicated Nurse Practitioner and Physician Assistant support through the Office of Advanced Practice Established mentorship and orientation programs available for newly hired providers Compensation Employed salary with benefits. #J-18808-Ljbffr
