Hospice Registered Nurse

Job Overview Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice to provide end‑of‑life care to patients in their homes in the Wilkes Barre, PA area. The position is full‑time, 40 hours per week with an 8‑hour shift and is ongoing. Responsibilities Deliver compassionate, patient‑centered care in a hospice setting, collaborating with an interdisciplinary team focused on comfort, dignity, and quality of life. Perform comprehensive patient assessments, symptom and pain management, and individualized end‑of‑life care planning. Educate and support patients and families through the end‑of‑life journey, ensuring they feel informed, prepared, and deeply cared for. Think critically, solve problems in real time, and maintain thorough, compliant EMR documentation. Work independently while traveling to patient homes within an assigned territory; never work alone. Two years of RN experience with the clinical expertise to deliver quality patient care; hospice experience preferred.
