Family Connections Consultant II
Critical Tasks
The Family Connections Consultant (FCC) provides homebased and community services in Maricopa and Pinal Counties to families referred by the Department of Child Safety (DCS) to participate in the Family Connections program. Family Connections is an evidence-based practice and includes, but is not limited to, the following responsibilities:
- FCC will attend in depth training provided by DCS to help families identify strengths and areas of need, empower families toward solutions, and alleviate the reasons for DCS involvement.
- FCC will provide direct service to program participants including outreach and engagement, assessment, personalized service planning, tailored interventions based on assessment and service plan, coordination of care and collaboration with DCS and other involved services, and guiding families to obtain needed resources (i.e. housing, counseling, Title XIX or XXI services).
- FCC will conduct home visits once or twice per week at times that are convenient for the family, which may include evenings and weekends.
- FCC will observe the children in the home once per week and report any suspected child abuse or neglect to DCS.
- FCC will promote positive parenting skills and recommend referrals to the Nurturing Parenting Program, or other parenting programs that meet the needs of the family.
- FCC communicate directly and regularly with DCS Specialists and outside agencies and services regarding participants’ needs and progress in order to ensure coordinated services that best meet the family’s needs.
In addition to the tasks above, the Family Consultant II will:
- Review reports written by team members and identify needed corrections.
- Upload required reports into Guardian
- Assist in the onboarding/training/scheduling of new hires
Key Performance Indicators
- Working knowledge of the child welfare system.
- Familiarity with skill building, psycho-educational work and motivational-interviewing.
- Knowledge of techniques and approaches to identify the child’s/family’s, adult’s underlying, unmet needs and can differentiate needs from services
- Ability to adjust schedule to the needs of the client which may include working some evenings or weekends.
- Completing all appropriate documentation within 48 hours of any and all communication with and regarding the family.
- Working knowledge of NextGen EMR system with minimal documentation errors.
- Participates in regular staff meetings and trainings as required by JFCS and our contractual obligations with the Department of Child Safety.
- All corrected and completed reports written by team members to be uploaded into Guardian within the timelines required by the DCS contract.
Metrics
- Successfully manage caseload of 4 to 6 families at any given time.
- Conduct 6-8 home visits per week.
- Participate in weekly individual and group supervision.
- Documentation must be completed in Electronic Medical Record system within 48 hours.
- Initiate contact with the family within one business day of referral receipt.
- Complete Family Assessments utilizing a computer assisted self-inventory and interviewing family members, extended family members, and other providers or services supporting the family within 30 days of referral. Complete an individualized service plan with UBSMART goals.
- Complete weekly reports for DCS regarding communication and progress for each family, which is to be uploaded to DCS website on a weekly basis.
Qualifications
- Bachelor’s degree in social service field required.
- One year of social service work experience required.
- One year experience as Family Consultant I
- Familiarity with the child welfare system strongly preferred.
- Experience doing skill building and psycho-educational work strongly preferred.
- Experience working with children and families strongly preferred.
- Reliable transportation required.
- Ability to obtain and maintain a Level 1 Fingerprint Clearance Card without restriction(s).
- Valid Arizona Driver’s License required.
- Active / Valid Arizona Auto Insurance with appropriate liability limits to meet contractual obligations.
- MVR Report that meets JFCS criteria for driving positions. MVR report is obtained prior to employment and bi-annually during employment.
- Ability to pass pre-employment and random drug screens.
- Must be able to perform the essential functions of the job including prolonged sitting and in certain positions driving, lifting 20lbs, walking, standing and executing dexterity with a keyboard or smart pad.
