We are currently seeking a Per Diem/Part-Time Nurse Practitioner or Physician Assistant to join our mobile care team. In this role, you will provide in-home health evaluations, build meaningful relationships with patients, and help bridge gaps in care. Role Overview The typical visit lasts about 45 minutes, allowing time to connect one-on-one, review medications and medical history, answer questions, and complete a physical evaluation. What Youll Do Provide compassionate, patient-centered in-home health evaluations Deliver comprehensive assessments, including medication review, medical history, and physical exam Engage with patients in their homes, assisted living, and senior community settings Build trusted relationships to support better health outcomes Travel locally within Los Angeles and Orange County Maintain accurate clinical documentation Communicate effectively with the care team as needed Job Highlights Pay-per-visit model flexible scheduling, you choose the number of visits Opportunity to work independently and create your own schedule Meaningful patient interaction with 45-minute dedicated visits Ability to make a real impact through personalized care Requirements (You can adjust based on your needs) Active NP or PA license in California Valid drivers license and reliable transportation Experience with geriatric or chronic care populations (preferred but not required) Strong communication and interpersonal skills Ability to work independently
