Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join as a traveling Physical Therapist providing home health services in Arbor Vitae, WI. You will help patients recover from injuries and manage chronic conditions while working across a rural territory that includes multiple small towns and lake communities. Expect to coordinate care in patients‘ homes and collaborate with interdisciplinary teams to support safe functional outcomes.

Minimum Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
• Active Wisconsin physical therapy license required to start the assignment.
• At least 12 months of home health physical therapy experience within the past 3 years. (Client requirement.)
• BLS certification from AHA or ARC may be required.
• Experience with HCHB electronic medical record preferred.

Preferred Qualifications

• Previous travel or agency home health experience.
• Experience managing a rural caseload and traveling between patient homes.

Assignment Details

• Shift Hours: 8am-5pm.
• Schedule: Monday through Friday.
• Territory includes Presque Isle, Manitowish Waters, Mercer, St. Germain, Eagle River, Land O Lakes, and Phelps.
• No on-call required.
• May require travel within the assigned territory between patient homes.
