Healthcare Professional OpportunityWe are seeking a dedicated professional to join our healthcare team. The candidate will be responsible for assessing the health of individuals and implementing their care plans, ensuring health maintenance, disease prevention, and providing case management. Supervision of care plans and staff members may also be involved in daily work assignments. Shift RequirementsMonday-Friday 8am-5pmOn Call Schedule: 1-2 days during the week and may have a weekend a monthLicensing and RequirementsRN compact state license requiredHome Health experience, HCHB and OASIS experience requiredSkills in IV, wound care, PICC line, wound vac, Trach, lab drawsResponsibilitiesObserve, assess, plan, implement, and evaluate nursing support for well, chronic, or acutely ill individuals in a community or small facility settingWork closely with other health professionals/providers in planning, implementing, and evaluating programs and servicesCoordinate communications and actions of various disciplines as it relates to health and safety supportsEnsure the ordering, reviewing, monitoring, discontinuation, and, if necessary, the disposal of medications as prescribed by the physicianEnsure policies are in place for medication administration, errors, storage/access, and destruction, and review them annuallyReport progress to the physician when the client’s condition changesTranscribe or ensure transcription of physician phone orders and other medically related reportsComplete daily, monthly, quarterly, and annual nursing documents as requiredDocument individual’s health status in nurse’s notesMaintain furnishings, equipment, and supplies to prevent the spread of diseaseDirect and supervise the activities of licensed practical/vocational nurses as neededSchedule and complete health-related appointments and implement appropriate responsesMaintain communication with inpatient nursing and discharge coordinatorsProvide new employee training and annual recertification training for health and safety topicsTrain unlicensed staff on person-specific health maintenance, safety, and disease prevention needsAdditional InformationProductivity Requirement: 20-30Mileage reimbursement: 43 cents per mileCounties covered: Clinton, Cumberland, and Monroe40 hours not guaranteed
