Job Overview WellMed, part of the Optum family of businesses, is seeking a Registered Nurse RN Home Health to join our team in San Antonio, TX. This field‑based position requires traveling around San Antonio and surrounding counties supporting Homecare Dimensions' patients. The RN will provide skilled nursing services and coordinate home health care services, ensuring quality and safe delivery in compliance with all applicable laws, rules, regulations and policies. Primary Responsibilities Perform nursing assessments and develop, implement and evaluate plans of care in conjunction with the physician, patient and caregivers per agency policy and procedure. Evaluate the effectiveness of the plan of care, the patient's response to care and outcomes, and revise the plan as needed. Re‑evaluate patient nursing needs and conduct ongoing assessments using the OASIS data set at appropriate time points. Provide skilled nursing services requiring specialized care, including medication administration, wound care, therapeutic treatments, and skilled teaching as ordered and indicated. Plan and implement patient health teaching and counseling regarding disease processes, self‑care techniques, prevention, and involve significant others in health goals to promote independence. Participate in health promotion and disease prevention activities. Initiate appropriate preventive and rehabilitative nursing procedures following the physician's plan of care. Coordinate services and scheduling with Clinical Team Manager (CTM) and Clinical Team Coordinator (CTC) and recommend additional home health care services within 24 hours of Start of Care (SOC). Assess the patient's condition at each visit, ensure assessments are communicated to CTM daily, and implement interventions to meet patient needs and respond to changes. Perform care‑management duties for assigned patients: notify physician of changes, obtain orders, reassess for recertification, supervise LVNs and aides, attend and document case conferences, coordinate care, and plan for discharge. Document all communications with the patient, family, physician, CTM, pharmacy, and other disciplines in communication notes. Prepare written plans for Home Health Aide when applicable. Express verbal and written communication in a clear, positive and cooperative manner. Work Friday‑Monday shifts and fulfill required on‑call schedule during weekdays and weekends. Mileage reimbursement and shift differential are offered. Required Qualifications Graduate of an accredited nursing school. Current unrestricted Texas RN license or compact license. BLS and IV certification. 1+ year of experience in home health care or related environment. Wound care experience. Proven computer skills. Ability to move and transfer patients. Driver’s license and reliable transportation to travel within designated area. Valid tuberculosis screening and documented evidence of immunity to measles, mumps, rubella, varicella, tetanus, diphtheria, and pertussis. Preferred Qualifications Bilingual (Spanish). Benefits & Compensation Hourly pay ranging from $28.94 to $51.63 based on experience, education, certifications, local labor market.
