TravelNurseSource is working with United Health Care Staffing, Inc. to find a qualified Home Health RN in Bardstown, Kentucky, 40004!

Location: Bardstown, Kentucky

Contract Type:  Travel Contract

Shift Type:  8-hour day shifts with weekend rotation, typically 8:00 AM - 5:00 PM

Hours per Week:  40

Candidate Pay Rate:  $46 per hour

Roles & Responsibilities

• Manage a caseload of roughly 30 adult patients in their homes
• Perform Start of Care assessments, recertifications, and daily visits
• Coordinate patient care plans and collaborate with interdisciplinary teams
• Document patient progress using Homecare Homebase (HCHB) system
• Educate patients and families on health management and home safety
• Provide skilled nursing care following physician orders and clinical guidelines

Qualifications

• Active RN license in Kentucky (or compact) required
• Minimum 1 year of Med-Surg experience in a hospital setting
• At least 1 year of Home Health experience mandatory
• Proficiency with Homecare Homebase (HCHB) system required
• Current BLS certification from AHA required
• Valid driver's license and reliable transportation (no mileage reimbursement)

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Excellent time management and organizational abilities
• Comfort with independent decision-making in the field
• Proficient in electronic health records, specifically HCHB
• Exceptional communication and patient education skills
