*ABOUT KEYSTONE PHARMACY NEW YORK*

Keystone Pharmacy New York, LLC is a licensed 503A traditional compounding pharmacy located in Albany, NY, specializing in USP 795 nonsterile compounded preparations for wound care patients. We partner with clinicians, wound care centers, and home health agencies to deliver customized topical formulations that support optimal patient outcomes. *POSITION SUMMARY*

The Pharmacist-in-Charge (PIC) serves as the senior clinical and operational leader of Keystone Pharmacy New York. This individual is responsible for all aspects of pharmacy operations, ensuring full compliance with New York State Board of Pharmacy regulations, USP 795 standards, and applicable federal requirements governing 503A nonsterile compounding. The PIC will lead compounding quality oversight, supervise pharmacy personnel, and drive excellence in clinical services for our wound care patient population.

The PIC reports directly to the VP of Northeast Market and delegates operational responsibilities to pharmacists, pharmacy technicians, and support staff.

*KEY RESPONSIBILITIES*

*Compounding & Clinical Operations*

* Oversee and perform USP 795 nonsterile compounding, ensuring all preparations meet identity, strength, quality, and purity standards
* Review and approve Master Formulation Records and Compounding Records for all nonsterile preparations
* Ensure proper selection, handling, and storage of Active Pharmaceutical Ingredients (APIs) and excipients
* Consult with wound care providers and other healthcare professionals to develop and optimize patient-specific compounded formulations
* Serve patients directly as needed, providing counseling and clinical guidance related to compounded wound care products

*Quality & Compliance*

* Maintain full compliance with USP 795, New York State Board of Pharmacy regulations, NABP standards, ACHC standards, and all applicable federal and state laws governing 503A compounding pharmacies
* Participate in the Quality Management Committee to identify, address, and prevent quality issues
* Follow, review, and approve Standard Operating Procedures (SOPs) and pharmacy policies
* Ensure accurate maintenance of prescription records and compounding records
* Immediately report and address prescription errors in accordance with pharmacy policies and the Quality Improvement Program
* Ensure all pharmacy permits and personnel registrations/licenses are current and appropriate for the pharmacy operation
* Verify that all pharmacy staff are properly licensed or registered with the New York State Board of Pharmacy

*Personnel & Operations Management*

* Supervise and provide ongoing performance feedback to pharmacists, pharmacy technicians, and support staff
* Conduct staff training on applicable laws, regulations, USP 795 standards, SOPs, and accreditation requirements
* Mentor pharmacy students and interns as applicable
* Monitor and coordinate daily workflow within pharmacy operations
* Oversee inventory management including ordering, receiving, verifying, and storing pharmaceutical supplies and compounding ingredients
* Maintain a clean, organized, and compliant pharmacy environment appropriate for nonsterile compounding
* Ensure accurate billing practices in compliance with payers, CMS, and applicable insurance programs
* Prepare and maintain daily operational reports
* Model punctuality and professional conduct as a role model for all pharmacy personnel

*Leadership & Community*

* Maintain regular communication with the Senior Management Leadership Team and keep leadership informed of operational issues and outcomes
* Participate in community outreach activities when available and appropriate
* Assume VP Northeast Market responsibilities in the absence of the VP Northeast Market, as directed

*Competencies*

* Deep knowledge of USP 795 nonsterile compounding standards and 503A pharmacy regulations
* Excellent written, verbal, and interpersonal communication skills
* Ability to develop training materials and deliver staff education on regulatory and clinical topics
* Commitment to patient-centered care and a collaborative team environment
* Proficiency with pharmacy management systems and electronic health records

*Education*

* Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy (RPh) from an ACPE-accredited institution

*Requirements & Qualifications*

* Minimum 5 years of experience as a Pharmacist-in-Charge, Pharmacy Director, or Clinical Director
* Current, unrestricted pharmacist license issued by the New York State Board of Pharmacy (required)
* Demonstrated experience with USP 795 nonsterile compounding; wound care compounding experience strongly preferred
* Knowledge of 503A compounding pharmacy regulatory requirements at the state and federal level
* Must maintain continuing education requirements for all applicable state licenses
* Must complete annual training in OSHA, HIPAA, Fraud/Waste/Abuse (FWA), and pharmacy policies and procedures
* Must not appear on the OIG/HHS exclusion list

Expected salary range: $140,000 - $155,000

Pay: $140,000.00 - $155,000.00 per hour
