Responsibilities
- Assess patients' functional abilities and develop individualized treatment plans to address ADLs, mobility, and cognition.
- Implement therapy interventions to improve independence and safety, with progress tracking and plan adjustments as needed.
- Collaborate with an interdisciplinary team to coordinate care, set goals, and support discharge planning.
- Document sessions, progress notes, and outcomes in accordance with regulatory and facility standards.
- Educate patients and families on home exercise programs, safety, transfers, and equipment use.

Requirements
- Active Occupational Therapy license in Maryland or eligibility to obtain Maryland licensure.
- Ability to work in a skilled nursing/acute care setting and participate in a team-oriented environment.
- Strong communication, documentation, and time-management skills.
- Availability for the 08:00-16:30 shift and travel assignment requirements.
