Responsibilities

• Provide physical therapy services under supervision, following the plan of care developed by the supervising PT.
• Implement and adjust treatments for LTAC patients, including gait training, range of motion, strength, and activities of daily living.
• Monitor patient progress, document responses to treatment, and communicate updates to the PT and interdisciplinary team.
• Maintain accurate documentation in the patient record and ensure compliance with regulatory and facility policies.
• Educate patients and families on exercises, safety, and home exercise programs; promote patient-centered care and empowerment.
• Participate in rounds and care coordination with the nursing, therapy, and rehabilitation teams; assist with equipment maintenance and infection control.

Requirements

• Active Physical Therapy Assistant license in New Hampshire or eligibility to obtain licensure per state requirements
• LTAC or skilled nursing setting experience preferred
• Availability to start on 11/16/2026 and commit to a 13-week travel assignment
• Ability to work the shift of 08:00–16:30 with guaranteed 35 hours per week
• Strong communication, documentation, and collaborative skills for interdisciplinary care
