Typical responsibilities include performing comprehensive assessments, developing individualized care plans, implementing therapeutic exercise and mobility programs, educating patients and caregivers, and evaluating home safety to reduce fall risk. You will manage a caseload that can include post-acute, orthopedic, neurological, cardiopulmonary, and chronic disease patients, focusing on functional improvement, independence, and quality of life.Your day will involve traveling between patient residences, completing timely documentation, and coordinating with an interdisciplinary team that may include nursing, occupational therapy, speech therapy, social work, and home health aides. Visit volumes are structured to balance productivity with safe, high-quality care and adequate documentation time. You will use an electronic medical record system and mobile technology to streamline charting and communication while in the field.The home health environment offers a high degree of professional autonomy and allows you to see the direct impact of your interventions in the patient's own setting. You will often work closely with families and caregivers, tailoring interventions to the home environment and prioritizing practical, functional goals that matter most to patients.
