Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Collaborate with interdisciplinary teams to develop patient treatment plans.
• Organize and participate in family meetings to ensure comprehensive care solutions.
• Coordinate patient discharge planning and resources, including DME and SNF arrangements.
• Ensure accurate and timely documentation using Epic charting system.
• Float between similar units as needed to meet department demands.
• Work weekends based on department scheduling requirements.

Qualifications

• Minimum of 2 years of case management experience in a clinical setting.
• Epic charting experience required.
• Current BLS certification.
• Experience with CMS guidelines and ICD-10 coding preferred.
• Demonstrated ability to manage a patient ratio of 20:1 effectively.
• Experience with community hospital settings and trauma levels preferred.
