This is a full‑time opportunity at our Racine location. $8,000 Sign On Bonus! Job Responsibilities Completes discipline‑specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults with families and other supports for additional information as needed. Develops and maintains care plans, including members' long‑term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services. Complete all required documentation and charting and assists in the development and maintenance of member records. Job Requirements Education: Associates degree in Nursing required, Bachelors Degree preferred. Licensure: Wisconsin Licensed Registered Nurse. Licensure: Eligibility for long‑term care functional screener certification is required. Licensure: WI Drivers License. Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity. Preferred 2 years of work in community nursing.
