This role is primarily remote, allowing for flexibility in managing your schedule while conducting home visits. • Conduct thorough assessments of seniors, evaluating their emotional, mental, and social needs to identify appropriate interventions.

• Develop and implement individualized care plans, collaborating closely with clients and their families to align with their personal goals.

• Provide empathetic counseling and emotional support during life transitions, helping families navigate the complexities of home care.

• Advocate for client rights and access to essential community resources, connecting them with necessary services to improve their well-being.

The Home Care Social Worker will primarily work with seniors aged 65 and older, many of whom face health and mobility challenges and require assistance with everyday functions. Clients may also include individuals with chronic illnesses or disabilities seeking support to enhance their emotional and social well-being. Your role is crucial in fostering their independence and providing them with access to resources that can help them navigate their circumstances.

• Bachelor's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Social Worker (LCSW, LMSW, or similar).
