RN Case Manager - Home Care

For experienced RNs seeking a career transition, part‑time home care offers an excellent entry point with a clear path to a full‑time Director of Nursing (DON) role. Position Responsibilities Summary Case manager is a Registered Nurse who is responsible for the overall direction of home care services. Case manager will have overall responsibility for the quality, adequacy of services provided and supervision of staff. Case manager establishes, implements, and evaluates goals and objectives for home care services that meet and promote the standards of quality. Case manager assures that care is coordinated between services and that all of the client's needs are identified through the assessment process and addressed through the implementation of the Plan of Care and supervisory process. This position will not perform any medical or skilled services. Services provided will be non‑medical. Directly Supervises Field Staff (Home Care Aides/Caregivers) Key Leadership, Management, and Accountabilities (LMA) Oversight and execution of client plan of care. Meet or exceed joint commission accreditation standards. Responsibilities 1. Provides non-medical care as needed. 2. Makes the initial evaluation visit and regularly reevaluates the client's needs. 3. Initiates the plan of care and makes necessary revisions based on feedback from care provider (HCA/Caregiver). 5. Initiates appropriate preventive procedures based on care provider's feedback. 6. Prepares summaries of care. 7. Informs personnel of changes in the client's needs. 8. Counsels the client and family in meeting related needs. 9. Assists in the development of organization goals. Develops, recommends, and administers agency policies and procedures. 10. Assesses competency of staff and ensures staff is assigned in a manner that promotes quality, continuity and safety of a client's care. 11. Supervises, educates and coaches care‑giving staff. 12. Plans and implements in‑service and training programs. 13. Conducts performance evaluation of care‑giving staff. 14. Oversees the maintenance of client records, statistics, reports and records for purposes of evaluation and reporting of organization activities. 15. Assures proper maintenance of records in compliance with local, state and federal laws. 16. Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel. 17. Identifies and participates in performance improvement activities. 18. Stays informed about changes in the field of home care. Shares information with appropriate organization personnel. 19. Promotes home care referrals in the community. Qualifications 1. Registered nurse with current unencumbered licensure to practice professional nursing in the state in which the care is being provided. 2. Preferred: Bachelor's degree in Nursing. 3. Minimum five (5) years of clinical experience with at least half of that time working in home care or related home care organization. Minimum three to five years of direct employee supervision. 4. Demonstrated ability to supervise and direct staff. 5. Comply with accepted professional standards of practice. 6. Proven ability to build quality and implement improvements within the operation. 7. Excellent interpersonal skills. 8. Analytic/Critical thinker. 9. Excellent customer service skills and ability to deal tactfully with clients, staff and community. 10. Treat clients, staff and the public with courtesy, respect and presents a positive public image. 12. Case management skills are a plus 13. Policy development is a plus 14. Understands regulatory compliance 15. a. State requirement for the franchise location 16. b.
